Due to the presence of codeine, Co-codamol is generally only prescribed when simpler pain relief options—such as paracetamol or non-steroidal anti-inflammatory drugs (NSAIDs)—have not provided sufficient relief. It is commonly used for managing moderate pain, including that associated with migraines, dental issues, and musculoskeletal conditions.

Codeine is a mild opioid analgesic that binds to µ-opioid receptors within the central nervous system, particularly in the brain. This binding alters the perception of pain and increases pain tolerance. Like other opioids, codeine may cause drowsiness, sedation, and respiratory depression due to its depressant effect on the CNS.
Paracetamol (also known as acetaminophen) works primarily within the central nervous system, where it inhibits cyclooxygenase enzymes (mainly COX-2), leading to reduced prostaglandin synthesis. Additionally, it influences several neurotransmitter pathways—including serotonergic, opioid, and cannabinoid systems—which further enhances its analgesic properties.
